Naturopathic Intake Forms
During your first visit, we get to know more about you, your health history and the reasons that brought you to see us. We go over a lot of information so it’s nice to be prepared! Please fill out the appropriate forms prior to your initial consultation and bring them in with you to your visit. They likely will take 15-20 minutes to complete.

What to expect for your first Naturopathic visit:
Naturopathic Medicine is a process. It takes time (and effort) to become healthy. The more you put into your health, the more you will get out of it. Our aim is to support and guide you through this journey and foster a long-term relationship with you.
Your first visit will be 1-1.5 hours long. This time allows us to get to know you as an individual and familiarize ourselves with your unique needs and concerns. During your initial visit we will:
- Discuss what brought you to the clinic, your goals, your past medical history and your current state of health (including physical, mental, emotional and spiritual aspects of health).
- Review any medical records or laboratory tests you have (you may obtain these from your family doctor or specialist in advance or we can have you complete a release of records and request past tests to discuss during your follow-up visit)
- Complete a complaint-oriented physical exam
- Begin initial treatment, which can include acupuncture
Please be aware that Naturopathic Services are not covered under OHIP. They are, however, covered by most extended health care plans. Be sure to check with your extended health care provider to determine the amount of coverage you have. All fees are to be paid at the time of service. We issue receipts that can then be submitted to your insurance provider for reimbursement as per your plan.
